Your feet are the first point of contact with the ground on every step. How they absorb shock, roll inward or outward and push off can change the way forces travel up through the ankle, knee, hip and lower back. That is why some people with knee, hip or back pain ask whether their feet could be part of the problem, and whether custom insoles in Delhi could help.
The short answer: sometimes foot mechanics contribute, and insoles may be a useful part of a treatment plan. But knee, hip and back pain have many causes, and insoles are never a substitute for a proper medical diagnosis.
The lower limb works as a chain
When the foot lands, the arch naturally flattens a little to absorb shock (pronation), then stiffens again to push off (supination). This movement is linked to rotation of the shin and thigh.
- If the foot rolls in too far or too long, the shin and thigh may rotate inward more, which can change load at the front and inside of the knee and at the hip.
- If the foot stays rigid and rolls out (common with high arches), less shock is absorbed at the foot, and more may be passed up the leg.
- A difference in leg length can tilt the pelvis and change load through the hips and lower back.
- Painful feet themselves change how we walk. People limp or shorten their stride to avoid pain, which can strain other joints.
Conditions where the foot connection is often discussed
- Pain at the front of the knee (patellofemoral pain), common in runners and people who climb stairs a lot
- Knee osteoarthritis affecting mainly the inner or outer part of the joint
- Outer hip pain and iliotibial band irritation
- Some types of non-specific lower-back pain linked to prolonged standing or walking
Research on insoles for these problems shows mixed results: some people notice a real difference, others do not. That is why assessment matters.
See a doctor first when…
Please seek medical review before considering insoles if you have:
- Knee, hip or back pain after a fall or injury
- Swelling, locking, giving way or a hot, red joint
- Back pain with numbness, tingling or weakness in the legs, or problems with bladder or bowel control (this needs urgent attention)
- Night pain, fever or unexplained weight loss
- Pain that is getting steadily worse
How a Footscan fits in
At P.O.R.C Safdarjung Enclave we use a Materialise Footscan pressure plate. You stand and walk across it, and the system records pressure under each part of the foot and the path of your centre of pressure from heel to toe. We also look at your legs, hips and posture while standing and walking, and at your shoes.
A foot scan is not a diagnosis. It cannot tell you why your knee hurts. What it can show is whether your feet load unevenly, roll in or out markedly, or differ from left to right, clues that help decide whether an insole is worth trying.

What a custom insole may do
Depending on the findings, a PHITS 3D-printed insole can be designed to:
- Support the arch so the foot does not collapse as far or as fast
- Add a small medial or lateral wedge to shift load at the knee (used selectively, for example in some knee osteoarthritis cases)
- Add cushioning for shock absorption, especially with high arches
- Add a small heel raise for a minor leg-length difference
The aim is to change how load is shared, not to "fix" the knee, hip or back. Insoles work best combined with strengthening exercises, weight management where relevant, activity changes and the treatment your doctor or physiotherapist recommends.
Realistic expectations
Give new insoles a few weeks of gradual wear. We review comfort and symptoms, and adjust if needed. If there is no benefit after a fair trial, that is useful information too, and we will tell you honestly. For knee osteoarthritis, an unloader knee brace may be another option to discuss with your doctor.
